End of Life Care Coordination Service Launch

The end of life care coordination service was launched on 5 July in Bristol. It is a new service for people who are in their last twelve months of life or less and will support patients, their carers, family members and key workers to ensure that they have access to the right services at the right time to meet their health needs and preferences. The end of life care coordination service does not directly provide care to patients, but coordinates available services in Bristol to support them.

The service will work with the patient’s key worker, whether this is GP, community nurse, or hospital healthcare worker, to ensure that patients have a package of care that supports their needs and also their preferences at the end of life. It also has the capability to support healthcare professionals with advance care planning, and communication, where the key health worker
requests this. It also provides a 24/7 telephone support service to patients, carers and families. Anyone who is assessed as being in the last twelve months of life or less is eligible to be referred to the service.

Eligibility Criteria
Patients referred to the end of life care coordination service must fulfil the following criteria:

  • Be 18 years of age or older
  • Be assessed as being in the final twelve months of life or less
  • Be registered with a GP in the Bristol area

Patients can be referred to the service regardless of their diagnosis and referrals can be made by any health or social care professional. The patient’s GP must always be informed of any referral made to the end of life care coordination service.

Referral Process
Once the patient has been assessed as fulfilling the eligibility criteria above, the referrer should download the referral form on the NHS Bristol end of life care webpage: http://www.bristol.nhs.uk/Staff/end_of_life/Bristol/default.asp

The completed referral form should be faxed directly to 0800 31 34 543.
